SAS Promotions Credit Card and Security Agreement

It is our policy to secure a signed Credit Card Approval Form for first time orders until our credit department reviews and approves all new credit applications. Therefore, SAS is requesting this credit card form be filled out and returned (via fax or e-mail) prior to the first order being placed.

I hereby authorize SAS PROMOTIONS to accept telephone orders and faxed orders from our business, and charge the required amount of such orders to my credit card account listed above.

I verify that all information provided is correct, and that I, the undersigned, am the authorized signer of the above credit card. I further verify that the signature below is my signature as indicated on the reverse side of the above-mentioned card. I hereby authorize SAS Promotions to charge the credit card mentioned above, without an imprint.

I have read this entire agreement and understand that I will be held fully responsible for its terms and charges.
